deleted a field by accident

How do I add a question or a field to my intake form?  it should be nest to DAte of birth but it's blank with blank boxes to check???

The question was deleted and the four answers as wll and it's a required question.  why can't i add in the test.

Answers should be married, single, widow, ect.
