Form Errors

Hello, I have a number of customers saying that they can't click 'Next' on the form - https://bit.ly/2PQQldN  - is there a reason / work around for this?

..is it possible to create an editable PDF to send to customers instead?

Upon testing the above link I am able to click most of the next buttons up to the submit point. The main problem I have observed in your form is that the "Date Field" (Session Date) might giving your user some confusions on the inputs they are going to make, if the user don't input the correct format of the date field, since it is marked as required, any wrong format inputs or empty field, they won't able to click the next button or make such progress until they corrected their inputs, this mainly causes the errors in your form.

I would suggest you to pre-populate the date field with the current date, this might lessen the confusions among user, they will have the idea to edit this field according to their desired date and giving them the idea what is the format of input in this field.
