My form will not consistently integrate with Google Spreadsheet

The first two form submissions auto popped up. Now it won't. 

Please note that making changes to the form or to the integrated sheet can break the integration.

In order to fix this, please remove the current integration and add a new integration.

The newly created spreadsheet will contain all the submissions and continue to sync the new submissions.
