Not receiving responses

I have created a form and unfortunately I am not receiving any responses. Could someone please look into this please.

I can see your form has been created under a guest account, upon checking the form in the builder it seems like it does not have an email notification, you may set up one following this guide: https://www.jotform.com/help/25-Setting-up-Email-Notifications

And provide us the username so we can move the form to the account and you can set up the email notification.
