Card form embedded on WordPress doesn't show on iPhone

We are experiencing an issue where our card form isn't showing on iphones.

We are using the standard JavaScript to embed it;

It can be seen while loading, but then it disappears.   

Am unable to replicate this error on my end. The form is showing when tested on an iPhone 6S Emulator and am able to fill the form and submit it. I recommend using the Iframe Embed Method as this will minimize the code conflicts between Jotform code and your website's code.
