How to create a PDF template for form submissions?

    Hello - I'm attempting to create a printable Medical ID card template where the user would submit a regular HTML form that includes prescription into, emergency contact phone, allergies, etc, 

    The requirement is to configure the form output/data so the fields are displayed in a way that will allow the pdf to be printed and then physically trimmed by the user to wallet size that they can carry around. 

    Fill out HTML form --> Submit HTML form data --> Download and print PDF

    The link I provided below is to an ASP.net version I built. I'm building a new PHP version of this website and need to re-create this capability.

    Can this be accomplished with JotForm?

    Hello - Our PDF editor allows to create a template to generate PDF files with submission data: https://www.jotform.com/products/pdf-editor/ 

    When you create a template, it can be even attached to email notifications users receive: How-to-include-submissions-as-PDF-attachment-in-the-email-notifications

    Please give it a try on a test form. If you will need any help when setting this up, just let us know.
