we set up a hippa account and have been locked out. Please advise.

I checked on the account and I don't see that you have already converted your account to HIPAA. I can also see that the account is active and not locked out.
Please enable HIPAA from this page https://www.jotform.com/myaccount/data or follow this guide https://www.jotform.com/help/500-How-to-upgrade-to-HIPAA-Compliance.
