Fields not showing up in Salesforce

    Hi  

    I have taken over a part finished project from someone who has now left. 

    We have made up a JotForm for each type of membership category, most information goes in fine, but the membership number is not showing in Salesforce.  When you search for that number in Salesforce it brings up the member information, but if you search for the member by name the field for ID (ie membership number is blank) and Salesforce does not allow it to be added manually because it is a duplicate number.

    In addition the membership category is not being transferred to Salesforce.

    I tried to add a pdf of the submission details, but I'm not able to attach, so I have attached the final page showing the member type and membership number.  After several attempts I've given up trying to attach a file!

    Any suggestions as to how to get this to work would be gratefully received.

    Hello Carolyn, Could you please name the form you are having difficulties with? 

    I see this one https://www.jotform.com/81847042771359 with SalesForce integration. But, it appears that SalesForce account is not authenticated:
