I created a form and realized I wasn't logged in. Is it gone forever?

I created the form below when I wasn't logged into my account. Is it gone now? Can it be transferred into my account? I can see the form still exists when I use the URL.

Additionally, I submitted a test and I do not know where the information will be collected and aggregated. Can someone please help?
