How to enable multiple selection in products?

Hi i am currently trying to add a few new areas to my form although am having a challenge. This first thing i am trying to add is an option to client can let us know when there opening and closing hours are and i have attached a screen shot of what i am trying to achieve. Secondly when i select a product on my form it does not allow me to select two products at the same time.  any help would be good;v

Could you please let us know to which form this issue is related to so we can look at it?
