How to create a scored test

    I would like to build a multiple choice test that has a point value (1-4) assigned to each answer. Is there a way to do this with a widget or something?

    Also,

    is there a way to save a form for later use? I use a form for my technician's daily checklist and when they close the browser it resets that form sometimes.

    What you are trying to accomplish is possible using the Multiple Choice field (commonly known as checkbox field). After you have placed the options in the field, enable the Calculation Value so you can set a value for each option which you will use later to calculate a score.

    After that, you can use the Form Calculation widget to get a total score — How-to-Perform-Form-Calculation-Using-a-Widget
