Need assistance in form configuration

Yes. I have tried my hands on one but the theme would not allow me to place
our logo. Can you assist? I will also like to add the course fee to the
form but there is no provision for that here.
I need help with the configuration of the response email. It looks a bit
complicated from this end and finally i want the a link before the submit
button that allows prospects to read the learning outcomes - a button
through which I can upload the programme outline.
