I'm trying to create a simple agreement form that has a field for client name, a start and end date, 2 pages containing the agreement and 3 fields at the bottom for printed name, signature and date. How can I do this?

    Sincerely,

    Howard

I have checked your account and I see that you have created a form of what it seems to be the kind of form you described in your requirements.

Since you would like the fields in your form to be part of the texts, I would suggest that you use the Narrative Fields widget in your form.

I created a Sample Form that is based on your requirements.

I suggest that you create a clone version of that form in your account so you can inspect it better.

As for separating the sections in your form into pages, please use the Page Break element of the form builder.

    Hi Mike:
Thanks for sending this. Is there a way that I can receive the full document with signature to my email after it is submitted by my client? This is important since it will allow me to print a copy.
Secondly, is there another option for signing the form other than the blue dot pen. This is not that workable. In other apps such as pdffiller there was an option for typing signature and have it appear in a scripted or handwritten format.
I’m seeing the title twice. Once at the top because it is the title of my form and then again below. What’s the best way of resolving this.
Lastly, the spacing between paragraphs is too great. Need to remove one line. Can I change this in the editor?
    Thanks for your help!
    Sincerely,

Yes, you may setup your form to send notification email to your email address when the form is submitted.

The notification email will have your form field data and it will show the signature.

    You may try these widgets.

    Docusign

    Adobe e-Sign

You may change your Page Title in your Form Settings.

Go to Settings>Form Settings>Scroll down until your find Page Title

May we know the form in question. May we know what paragraph are you referring to?

    Looking forward for your response.
