Permit Application leaves out information from submission PDF

The pdf version that is attached to all emails for Fence Permit Applications leaves out important information from the application. The part where the applicant chooses whether to have the permit mailed or come in and pick up is left out.

Check the body of the PDF to ensure the field is present in the PDF.  Here is how to edit the PDF:

If it is missing, add it in.  Alternately, you could create a new PDF, which would have all current form fields.
