Why do the survey buttons not light up on the website, while they do in the PDF

I believe you are referring to the checkboxes in the questions that you have selected above. By "light up", are you referring to the background highlight that appears when a required question is not filled?

If you are facing this issue with the form that you have embedded on your website, please provide us your webpage URL so that we could check accordingly.

Also, it is suggested that you embed your forms on your website using the iFrame code.
