how to fill out forms on ipad

Hey, I need a guidance here. I've hired a couple of people to conduct product surveys. I will give them an iPad and they will be doing surveys by visiting ppl in their homes. The problem is how they can fill forms on this device. I'm not looking for a pdf form or something like that. The second thing is that they will spread all over San Francisco, going different neighborhoods, and I want each to have specific forms so that we can understand how people in that neighborhood use the product. Appreciate any help.
