Unexpected error occured control_matrix in form

Hi, when building my form I added an input table and during the setup I got an unexpected error occurred control_matrix.

Now my form is corrupt.

Can you help please?

 I did check your form and it returned a form engine error.

I have cleared your form cache.

But it still returns an error at my end.

Can you please try reverting your form to the previous version?
