I can not see the background image

Just created a survey, but the background image works in preview form, but when I published it, the background image disappears.

Am I doing anything wrong? 

I have just checked your form (https://www.jotform.com/91692574903567) and it has a background. Is there another form where the problem occurs? If so, please provide screenshots, so we can further investigate.
