We are looking to get the custom form we built to be emailed as a PDF attachment but to remain in the similar format.

Do we need to create an additional pdf form to sync the two forms to work in sync with one another?

You will need to create a separate PDF file to add as an attachment to your form. It can either be a simple PDF file or a PDF form.
