Trying to create a form with multiple approvals

i am trying to create a form that will have several approval levels.  Each form submitted would have different approvals.

Example: A mother submits the form to have her child released from one team and go to another team.  She has to supply the coaches email for both  the releasing coach and the receiving coach.  All three have to approve/sign the form and then if everything is correct we would approve it. 

How would this be set up when you will always have different email addresses?

I have looked at that, the issue I have is each form submitted would go to what ever "Releasing Coach" and "Receiving Coach" email that was listed on the form.  They all will be different.

I have reviewed your form and the autoresponders that will be sent conditionally. As fast as I can see they should be sent to correct recipients when the emails are triggered. 

You probably need to use the following to make your form more accurate : 

1. Hide Coach Signature fields on the form so they are not visible when parents fill the form. You can show them on the form when the form is being edited by the respective coaches.

2. Get page URL widget: Use this widget so you can trigger your conditions when the form is being edited.

3. Add conditional logic to make you form more effective. E.g. Only show fields that are relevant to each user.
