Redirect to a different form based on user's answer

I have a series of yes/no questions and also a multiple question with about 6 possible answers.  based upon the answers selected, it is possible for the answer to send the user to a particular other form?

I basically would like to lead them through these questions such that they end up at a decision point with full explanations based on the answers that they gave along the way.
