Error with date field

We have several forms we use  that contain sensitive information that is required. We have had several people call our office now and tell us they can't complete forms with required date fields, they are getting an error "incorrect date" when trying to submit. 

This means we are having to make the field not required and ask for them to email the information separately across. When dealing with application forms or reference forms this isn't professional or ideal as we don't have all information in one place.

How can we resolve this issue? It's happened across multiple forms we use.

Could you please share the URL of the form? I checked your form 92032257524351 and see that you are using a short text entry field for the date. Are you facing issues with the short text entry field or you replaced the date field with short text entry as you are getting an error?
