Account Suspended

    Hi, my account got suspended and i dont know why. Im creating an online form for the clients of our company to upload their information and them store this in a database.

    Could you please reactivate my account? i'm tight on schedule and i really need access to finish setting this up.

    Hi Carlos!

    Your forms and account were suspended because you are collecting sensitive information such as passport numbers without signature and terms & conditions widgets on your form. You need to have these 2 widgets at all times on any forms which are collecting sensitive information:

    1. A signature widget - How-to-Add-an-E-Signature-to-Your-Form

    2. A Terms & Conditions widget - Short Scrollable Terms or Terms and Conditions

    We have already added these widgets to your forms and reactivated the account. You can edit the text of the agreements if you would like. Please let us know if you have any questions.
