Pre-fill a form for someone else to complete

I have been playing around with the card form using the Save Form and Complete Later option. That does not seem to be what I need it to do. I want to allow someone with access to the form, plug in a set number of fields, (Name, Position, Supervisor's Name), then generate a link so that they can send that form to the supervisor to complete the remainder of the form. It is an employee performance survey form. So in theory, we plug in the employee's name and other info, then send it to the customer for them to evaluate, but they need to start at the beginning of the form. When they are complete, they submit and it sends to us. How to do that?

Well, not quite what I need.  People who would have to pre-fill and send the form cannot get into the account to do what they need.  I am looking for a way to create a form, have person A fill in some of the info, send it to person b to complete and submit it.

I believe what you are looking for to create is something similar to an approval form.
