Can I create an order form with calculations for quantities and unit cost?

I tried to send a sample form (jpg) of what I want to create but it was rejected. I need to have a form that can calculate quantities and totals for a number of products. I need all the standard info such as "Bill To" and "Ship To" fields. Is this possible with JotForm?

Yes, what you are looking to do is possible with JotForm. You can either try to use the Input Table field or the Spreadsheet widget to represent the table shown in your screenshot.

As for the "Bill To" and "Ship To", you can use the Address field and you can also position the fields side-by-side.
