Form where thumbnail of image prints

Good afternoon. I am curious if there is a format that allows an image, that people upload when filling out a form, appear on the form when printing a hard copy.

You can create a PDF copy of the submission which includes the uploaded image. Then create an email autoresponder where you can attach the said PDF. 

So when the user submits the form, he will receive an email with the PDF as an attachment. And from there, he can print it out when necessary.
