We are a healthcare provider and we use JotForm for our patients to fill out their intake/health history forms. After they have submitted the form, we download the PDF and put it in their file. The PDF no longer includes the details of the consents that they are electronically signing for... it just includes their electronic signature. I have included screenshots of what I mean. We need for the PDF to include a description of what they are signing for. It used to but now it doesn't.

Please check the option to show the headers and texts on the submission, this can be done from the submissions page:
