Questions about thee plans

I made my first form. Is that form public or private? On the free form plan, you get 5 forms. Is that 5 separate forms? or can I only use the form I created five times? Also is that 5 forms/month or forever? Another question, how many times can I use my form?

You can share the direct link of your form with your users. So, it is public.

Yes, 5 separate forms. This is the number of forms you can create and that are active. You can enable and disable the forms to allow you to set which ones should be active.

Also is that 5 forms/month or forever?

This is a fixed limit. It does not reset. As I mentioned above, you can enable and disable forms, in order to free up space.
