PDF form is different from the online form.

    ok, so I've created a template from one of your new PDF forms, it's was the job confirmation form. When I click on the form, it's bringing up a completely different form....why though?

    Are you referring to this form https://www.jotform.co/form/70378303802856? Please note that you can manually add the fields that wasn't created when you created the PDF Form.
