I need to attach an downloadable pdf form

I need to attach an downloadable pdf form on our board of directors web site.  

How would I create that form so the person can complete the form and email it to one single address.  

I know how this works on a MAC but most people will be looking at the web on a windows PD or cell phone.

If you are referring to attaching the original PDF file on your form for the users to download it as a PDF file, you can use the PDF Embedder widget. Then the users will be able to download it using the 'download' button.
