Have you setup anything for Caterers that does weddings ?

We are looking at a covid-19 form to be filled out by wedding guests, when we are allowed to do weddings again. Since this is new to us I was wondering if you have done something like that- we would need to send a link for the event to the bride & groom and then they would forward it on to Guests to Sign a couple of days before the wedding. 

Once you create your form, you can simply get the form URL and send it to the bride & groom. They can then forward it to guests.
