how can upload photo on form

I am trying to create an admission registration where people submit their details and a photo to a fillable PDF that can be downloaded. 

I checked your form but could not find any PDF form/fillable PDF. Are you still trying to create one?

If yes, please note that it is not possible to have a file upload on a PDF Form.

However, you can add a File Upload or a Take Photo widget on your online form.
