How do I put my form on my Wixsite?

It is very unclear to me how to start with a pdf and make a form. I don't want to change the pdf and use that, I want to use the FORM based on the pdf. I want to embed this in a page on my Wix website. Your instructions don't make sense to me....

How do I get a CALL to work on this?? Back and forth on email is very impractical for a business owner.

I know how to remap things. My question is, after I am done redoing the form, how do I use THAT in my website, rather than the pdf.

Sorry for the confusion. You can embed the web form version in your Wix site using the Wix embed code. To get the embed code of your form, please refer to my screencast below.

You can also use the iFrame embed code located in the "EMBED" section.
