The embedded form is not responsive

    I had my website manager embed our form on our webpage under the registration tab.


    I can complete the first part of the forms, but the health history does not appear and the NEXT BUTTON does not work.

    Could you please re-embed the form using the iFrame code and let us know how it goes.
