How to edit form in Form Builder?

Hello,

I can't seem to figure out how to edit one of your forms that links from your website. I had a web developer who set it up that I can't seem to get hold of anymore. Is there any way I can gain control of it as it has information that is sensitive to our website? The link is located on this page https://www.behaviouralreferrals.co.uk/how-to-arrange-a-referral-for-your-patient/ and the form is here https://form.jotformeu.com/61872049328360

Is there anything that can be done?

Thank you very much for your quick response. We, unfortunately, have no access to that and have no idea what email that is associated with. Is there anything else that can be done to retrieve the control of that form?

Unfortunately, we cannot provide access to that account without the account owner's permission.

You may need to contact the form owner to gain access to the form.
