Having issues with form Submit Button

Hello, I’m having issues with my form called internal business referral form on my login. Here is her URL to our website where I have embedded the form but when you fill out the information and try to hit submit it does not work and I cannot figure out why.

I can see you have embedded it using its script code. The issue could be is that the scripts on your webpage might have conflicts with the scripts in the form. So embedding the form using its iframe code makes it load independently from your webpage.
