Please make sure that the column names on your sheet should be labeled exactly the same in your form.

Can you share with us a screenshot of your spreadsheet so we can check? Based on the image you've posted, your fields are labeled as "A" and "B", so you will have to add a separate column header with the same name "A" & "B" you cannot use the default headings (e.g. A, B, C,...Z) to match the field labels in the form.
