Create profiles for people who frequently submit the form everyday.

Hello,

We are a non-profit that provides thousands of free meals as a COVID-19 pandemic relief effort. In order for us to follow county requirements we need to capture demographic data.

Our sign-in sheet is in two parts. Part 1 is the sign-in form and part 2 is the demographic form. The demographic form only needs to be filled out once, while the sign-in form (Part 1) needs to be filled out everyday. Is there a method to pre-populate or "search profile names" for the the sign-in sheet for user profiles so we don't have to keep asking the same questions?

I asked this because this slows down our check-in process when the line is around the block.

You can use the spreadsheet to form widget so that they will just need to input the unique code and it will automatically fill the fields that they normally fill.

Just to let you know that the Widget only supports the following fields:

Full Name

Email

Time

Short Text Entry (Textbox)

Long Text Entry (Textarea)

Dropdown

Single Choice (Radio Button)

Multiple Choice (Checkbox)

Scale Rating

Star Rating
