Need some advice on how to make this form more manageable please

I run a youth football team with 16 teams and we are using Jotform to register players attendance for insurance and COVID record benefits.

The form we have set up asks if a player is in attendance and then asks if they have brought certain items with them and if the parent is staying.. link is included below.

The only way i can work out to make the questions required is to use the conditional settings that then shows the questions and makes them required if the player is tagged as attending.

If the player isn't attending the questions can't be required otherwise the form won't submit.

Is there a widget that would make doing this easier please. With 16 teams of 15 or more players setting this form up in this manner would take days.

Hello David,

Do you plan on adding more fields on this form - name fields perhaps for each player? I'm sorry, but I'm not exactly sure what's your problem on this form. The form should work the way it is currently set up.
