I am interested in purchasing a simple form, but I need my form

to point to 4 other jotforms for selection for what their problem might

be. Is it possible to talk to a live human?

You don't need to purchase a form.

You can create a custom form as per your requirement on JotForm in your account.

Do you want your form to point to four other forms after your first form is submitted?

If yes, you can use a conditional Thank you page feature to show the different forms based on any response in the form.
