Form submission email hiding fields

    Good morning!

    I created my first form and have set it live but it seems that some of the field entries are hidden on the email that is sent to us. For context, the form is an application for our animal shelter's clinic. When someone submits and application, we receive it via email, print it out, and it becomes part of their client folder. So we really need to be able to see all the questions, even if they don't answer them. As far as I can tell, I have everything set to NOT hide fields, and some of the fields it is hiding are required fields so I'm not sure how that's happening.

    I have included the URL to the form in question. For example, one of the questions that has been hidden on the email is "Does your pet lunge, bite, etc." BUT the "If yes please explain" field does show up.

    This is my first JotForm so I apologize if this is something simple! Please let me know how to troubleshoot. Thank you!!

    I am sorry for the trouble caused to you. It seems few questions are not included in your form's email alert. This happens when you edit your email alert and then add questions to your form. The best way to solve this problem is to delete the existing email alert from your form and add a new one. When you add a new email alert, it will always include all the questions of your form.
