Is it possible to integrate data from the table to the new form?

Hi, I would like to create rubric for students evaluation in form. I'm thinking of using combining dropdown (i.e. student's ID) and display their photo on top of the from. It would be great to identify the students and ID text.

Their ids, photos etc already collected in the table from previous submitted form. Is it possible to integrate data from the table to the new form? I'm tried looking at ddslick and dynamic form and seems not what i'm looking for. Rubric should be in form not report. Or is there other way around?

Just to clarify, you would like to populate the assessment form with data from a previous form?

However, with this, it is not possible to prepopulate an image field.
