How can I add this ICD-10 lookup?

    I am creating a CoVid form and I need a list of ICD-10 codes. The NIH has this lookup tool (see link under "API Demo"). We need the ICD-10 code to bill. Do you know how to add the lookup tool to the form?

    Do you have access to this third party application natively or simply through the Queries as listed in the link you sent?

    Unfortunately, it is not possible to add codes to the form.

    You can get the form source code and modify it by adding your codes - after that host it on your server. But every time the form is revised, the source code of the form will need to be downloaded.

    OR you can export the data as excel, use a Spreadsheet to Form widget on the form and then pass the codes to a field on the form.
