Do you have a feature that allows customers to schedule appointments after they complete a survey?

Hi, yes we do, you can add the Appointment field on your form, please check this guide: https://www.jotform.com/blog/how-to-schedule-an-appointment/
If you want to redirect the user to the appointment form from your survey form, please check this guide:
