Form modules are not working correctly

    Hello,

    I've embedded 4 forms to this website:

    • I had to remove all of the required questions to get them to submit when I tested them. This is especially true for the yes/no questions, and would like for those to go through.
    • The date picker isn't working.
    • The patient history form will not continue after entering my email.
    • The icons on the beginning of the forms are white and green, but I have them all set to white. Why is it interchanging?
