Electronic signature for the form.

Good afternoon,

I am a potential new customer for you, looking for a new electronic signature company for my physical therapy business. Two questions:

1. 1. could I use Jot Form for reimbursement forms, where I am the only one completing the form, but need an electronic signature before emailing to a patient?
2. You had a 50% off new subscription promotion in December. Would you be willing to offer that deal again?
