Can a new patient form be sent or linked-to following an appointment being made via Square Appointments?

How do most health care providers set up new patient forms? And does zapier -which I haven't yet used or learned to use- send info both ways? My hope is that new patients could be automatically sent new patient intake forms after making appointments through my online Square Appointments app and that the info on the form could be populated with the patient's information -but editable in case a parent or family member is making the appointment on behalf or a child or other family member. Any other tips and CRM integration ideas are greatly appreciated.

How do most health care providers set up new patient forms? — We don't have integration with Square Appointments, only Square payments. You might want to try Google Calendar which you can easily organize your new appointments via Google Calendar app.
