Form with PDF is not showing

but when i try opening it or preview it, it shows up blank

The issue is due to the condition below in your form:

Yes, that is possible. What you need to do is to create an approval form, so please refer to this guide: https://www.jotform.com/help/432-A-General-Approach-on-Building-an-Approval-Process-Workflow

You have the first signature then the other person will receive a notification email with the Edit Link and he will add his signature.

It has a signature field included as well.
