How to let the user register multiple times in a form?

    We have a project at an Airport. We are doing Covid testing for employees and then general public. Right now, they are using a company that used Jotform to collect pre-registations. We need to be able to allow people to:


    Pre register within designated times. 3 times a week within a 2 hour window

    check off a consent form

    take that information and download or upload it to integrate it into a Lab Information system so that the lab can pre-print a label for the persons' testing tube. Bottom line is the the information from Jot form has to get into the Lab' software program

    Here are my answers to your questions :

    Your mind that the person would have to submit the form 3 times per week.

    Yes, it is possible to achieve your requirement by taking advantage of our "Edit Link" feature. Here are the steps you need to follow to achieve your requirement:

    #1. Please add the "Submit" button on each page of your form.

    #2. Edit email alert of your form and add "{edit_link}" tag in an email template.

    #3. Please ensure that the "Send on Edit" setting is enabled in your email alert template. Please check the screenshot below:

    When the form is submitted the first time, he will receive an email that will contain a clickable link. When he clicks on the edit link, he will be able to go to the same form where he can input data again.

    You can also add a reminder and schedule a time to remind the person to fill out the form :

    Check off a consent form

    I'm not sure if I understand correctly your question. We have some widgets that can be used as consent, such as the E-signature. and also the terms and conditions widget.

    Also, in the calendar widget, I set it up for interval every other day but the calendar in the form is showing every day of the week.

    I assume that you are referring to this form, 210374480385052, correct? But as I have verified, the Intervals is set to Weekdays, and you can try to select which day of the week you would like to make the appointment slots available:
