Yes, it is possible to create a custom URL of a form. Custom Form URL is useful if you want to publish your form using the direct URL and want to put a custom name on it. You can add your own form name at the end of the URL along with your JotForm Username.

Regarding "We don't want to see the word JotForm anywhere."
If you want to use your own domain name, you can try the JotForm Enterprise https://www.jotform.com/enterprise/
instead of using the regular form URL (https://form.jotform.com/1234567890) you get after publishing your form, you can load the form with a customized domain: http://forms.mycompanydomain.com/1234567890
If what you mean is to remove the branding, than you need to upgrade to our paid plans, then you have to uncheck it from the settings page of your account.
